Estoy en la versión 10.11.1 de Mac e incluso con un sistema de archivos "sensible a mayúsculas y minúsculas" en una imagen DMG, obtengo:

Parece que hay un error en la aplicación de la Utilidad de Disco recién reescrita en El Capitán (a partir del 10.11.2) donde no puede crear imágenes de disco sensibles a mayúsculas y minúsculas. Como mencionó su respuesta, puede usar la herramienta de línea de comandos hdiutil como solución alternativa; alternativamente, en Disk Utilty, puede borrar la imagen del disco después de crearla, seleccionando un formato que distinga mayúsculas y minúsculas.
